CMWL Shop Terms & Conditions
CMWL offers various products in its online store, including recurring weekly or bi-weekly meal plans, and items for individual sale. These terms and conditions describe the policies of CMWL for each type of product.
Health Insurance Coverage
The nutritional supplements and healthy foods provided by CMWL are not covered by health insurance plans such as Medicare or Medicaid. You will not be reimbursed by your health insurance provider for purchases you make of these supplements or foods from CMWL.
Physician Incentive Disclosure
In the event that you are purchasing CMWL products as the result of a recommendation from a physician or other practitioner, such physician or other practitioner may receive a fee from CMWL in respect of such purchase, and other similar products may be available to you from other vendors. Please consult your physician if you have any questions regarding these acknowledgements and certifications.
Returns & Refunds
For all CMWL Doctor’s Orders meal plans and products for individual sale, there are no returns or refunds. All sales are final. If you receive a package that has been damaged during shipping, we will repackage and reship your order in an expeditious manner.
Doctor’s Orders Meal Plans
Recurring Plan
All Doctor’s Orders meal plans are recurring plans, meaning that we will continue to ship you meals until you cancel your plan. Recurring plans are offered in 7-day or 14-day options, depending on which plan you select. You may cancel or change your plan selections at any time. Cancelations and changes will not become effective until the week or next shipment following your last bill date.
Billing
You will be billed every 7 or 14 days, depending on which plan you select, until you cancel. Notification of your next bill date and amount can be found in your My Profile section of www.centerformedicalweightloss.com.
Payment Methods
Only credit and debit cards – American Express, Visa, and MasterCard - are currently accepted forms of payment. Orders will be fulfilled only upon successful billing of the credit card you use to purchase your Doctor’s Orders meal plan. If a charge to your credit or debit card results in failure, we will notify you on the site and/or via email, and provide you the opportunity to update your credit or debit card information.
Ordering, Meal Plan Changes, Cancellations
Doctor’s Orders meal plans are currently only available for order online using the secure checkout process on www.centerformedicalweightloss.com. You are required to set up a free registered user account in order to purchase and follow the plans. Changes to plan selections, including cancelations, may only be made online via your My Profile page. Any changes or cancelations will not become effective until the week or next shipment following your last bill date.
Shipment & Delivery
Doctor’s Orders meal plans are available for shipment to all 50 States. Meal plans ship Monday through Friday, with the exception of major holidays. Please allow up to 7 business days for delivery. Meals are shipped via UPS and contain dry ice to maintain product integrity during shipping. A signature is required for successful delivery.
Meal Products
You may select the types and quantities of the meal items within each meal plan offering, as long as the meal types and quantities fulfill the requirements of the specific plan you choose to follow. Exact calorie counts may vary slightly based on individual portion sizes. Nutritional information is located on www.centerformedicalweightloss.com for each meal item offered, including allergy and sensitivity information. The Center for Medical Weight Loss or any CMWL Physician cannot be held responsible or liable for any injury or illness that may arise as such disclosures have been made.
